Returning Patient Appointments

For returning patients and follow-up appointments please call 213-977-2450

We are open Monday to Friday (except Holidays) between 0830 am to 5:00 pm.

Please arrive 30 minutes early for your appointment to fill in a health questionnaire and complete the registration process.

On your appointment day, please remember to bring the following:

• Medicare card / Insurance card
• Valid photo ID
• Referral letter
• Any xrays or scans that you have on film or disc
• Copies of operation records, medical records, results and so on from prior doctor visits.
• If you have had surgery elsewhere, please bring a copy of your operation report.
• A list of current medications and any known allergies
• Completed registration form provided by our office.
